The distal tibiofibular syndesmosis is the articulation that lies between the tibia and the fibula at the ankle. Four ligaments — the anterior, posterior, and transverse tibiofibular ligaments and the interosseous ligament — are responsible for maintaining the stability of this articulation. Injuries of the syndesmosis are relatively rare but are frequently misdiagnosed [1]. Anterior-posterior and mortise radiography are often used to assist with diagnosis; however, they are not always accurate in identifying syndesmotic injuries, with success rates of 48% for anterior-posterior views and 64% for mortise views [2].
